Paperwork
First-time patients are required to complete health history and consent forms. To avoid eating into your appointment time download them below and bring the completed paperwork with you to your first appointment. If you forget or don’t own a printer, don’t worry. I’ll have forms available for you to fill out when you arrive.

First Session Overview
For your first session, we’ll spend 10-15 minutes going over your health history, doing posture and/or range of motion assessments, and discussing your primary areas of concern. Depending on what we’re working on and your personal preferences, sessions usually blend a variety of techniques.
I encourage all my patients to be as vocal and engaged as they’d like - if you want more/less of something, have questions, or just want to chill out please let me know. This is your reserved time and it should fit your needs and preferences.
At the end of your session, we’ll take a few minutes to note changes in your body and address any questions you might have. If you’re interested, we can also discuss self-care options for you to continue at home or to integrate into your workout routine.
Dress/Draping
Clothing level during your session is completely up to you. Many of my patients prefer to arrive in sports/gym clothes and I simply work over them, while others like to disrobe and get cozy under the heated blankets.
If you’re specifically coming in for pelvis/sacrum work I encourage you to bring or wear a pair of gym shorts/running tights. This can make measuring bony landmarks around the pelvis much easier.
Regardless of what we’re working on, sensitive areas (breasts, groin, gluteal cleft) will always remain covered, and I’ll never work on sensitive areas of your body that you haven’t given me permission to address.
Payments
I accept cash, Venmo, and credit cards for all services and certificates.
If you are planning to use your flex spending account/health savings account, L&I, or PIP claim please let me know before your appointment!
Insurance
If you plan to use any type of insurance or employer sponsored savings account, you must let me know before your appointment. I take a limited amount of L&I and PIP clients at a time, due to the increased paperwork and time necessary to manage these claims.

Covid-19
I take every precaution possible to keep myself and my patients healthy. I am fully vaccinated and boosted for Covid-19, and I continue to wear a N95 mask for all my appointments. Please read through my safety practices and agreements before coming in.
+ Safety Practices & Agreements
Under the current conditions, everyone must take all possible precautions to stay healthy and to protect the health of others. In order to currently treat patients, I must take into consideration my own health as well as the health of everyone else who comes into my office. I ask that each person who comes for treatment enter into an agreement with me and commit to following these protocols. Even with following best practices, I cannot guarantee 100% safety; there is risk—albeit low—in coming for treatment. These protocols may change in the future.

DO NOT COME IN IF YOU HAVE OR HAVE HAD WITHIN THE PAST 48 HOURS ANY OF THESE SYMPTOMS
- Temperature of 100 degrees or more, or 2 degrees above your normal. Your temperature should be normal, without taking any medications to lower it, for 48 hours prior to your appointment.
- New cough
- Sore throat
- Unusual fatigue
- New or unusual muscle aches all over
- Chills or shaking due to chills
- Diarrhea
- Loss of sense of smell or taste
- New or unusual headache
- Shortness of breath or difficulty breathing
- Chest pain

Ballard Office
My Ballard location is shared with BASE Chiro & Sports Therapy - a chiropractic and exercise therapy office. We’re located on NW 57th Street, a block north of Mr. Gyro’s (if you’re like me and orient yourself based on food). If the front door is locked when you arrive, give me a text (206-800-1030) so I can come get you. Please do not arrive more than 5 minutes early as I cannot guarantee that I’ll be available to let you into the building.
Parking:
Street parking is VERY limited close to the building. The nearest paid parking is located behind Bank of America (20th Ave & 56th St). Honestly, my best advice is to walk or bike if you're able! There is room inside the lobby to park your bicycle if needed.
Please bear in mind that the Ballard Farmers Market takes place every Sunday from about 9am-2pm, rain or shine. Parking and roads are cutthroat during this time, so plan accordingly.
